About Us
Faith Collaborative Hawai'i is a network of churches working together to meet needs of people in Hawai'i. Our desire is to see churches in Hawai'i working together in unity to share the love and compassion of Jesus. Member churches offer their programs independently as part of the Unite Hawai'i network, collaborating with each other and the community to provide coordinated care to the people being served.
Unite Hawaiʻi is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Unite Hawaiʻi is sponsored by Kaiser Permanente and AlohaCare. The network is supported by a Hawaii-based Unite Us team focused on community engagement, network health and optimization, and customer success.
